Who we treat
Our health services are open to adults and children of all ages.
New patients
Please complete a Health Inventory Form.
If you have a first-time appointment for homeopathy, medical massage, or neurofeedback, please print and complete this Patient Intake and Health Inventory Form (click to download and print).
Rescheduling appointments
Please notify us as far in advance as possible if you are unable to keep an appointment or need to reschedule. You will be charged for appointments that are missed or canceled less than 48 hours before the scheduled appointment.
Paying for services
Payment is expected at the time of your visit by cash, check, MasterCard, Discover or Visa.
Please note, for homeopathic and neurofeedback services: A $200 deposit is required when you call to make your appointment. This is a non-refundable deposit, however, you can reschedule your appointment if necessary.
